Dear teacher,
Please use a questionnaire to help us measure what your students have learned through the Healthy Kids Quest. You can also use the questionnaire as a tool to measure student performance in your province's health and physical education core curriculum.
Students are to answer the questions to the best of their knowledge. The questionnaire is to be completed a first time before the program starts, and then a second time after students have taken up the Healthy Kids Quest challenge.
Once the questionnaires have been returned to you, please complete the Evaluation for Teachers form online (below), or send it to us by email or postal mail:
